Payments
Payment for office services are due at the time of the visit unless prior arrangements have been made. We accept cash, checks, debit cards and major credit cards for your convenience. If we are a participating provider of your insurance carrier, we will bill them. However, co-payments, if applicable are due at time of the office visit. Ultimately, payment for services is the responsibility of the patient.
Prescriptions & Renewals
Please request all prescriptions and authorizations during business hours through your pharmacy. There are no meds on site.
